The Care Manager Transition Program is an inpatient based training program that includes both virtual classroom and precepted hours. The care manager will coordinate with the health care team and the patient/family to address payer issues that impact care and to develop a transition plan of care that includes a safe discharge plan and follow-up with care providers post-discharge. The care manager ensures that the patient/family agrees with and understands this transition plan and that it is implemented appropriately.
Program Eligibility Requirements*
- Two years of recent acute RN experience
- BSN preferred
- Current California RN license
- Case management and/or home care experience preferred
- BLS for Health Care Provider from the American Heart Association
- Case management certification highly preferred
Program Duration
- 8 weeks
Curriculum
The program uses a blended learning approach:
- Instructor-led lectures that focus on processes, skills demonstration, simulation exercises and case scenarios
- Clinical shifts with other specialty roles within the case management setting as well as site clinical shifts and preceptor assignments
- eLearning developed for case management and transition of care as well as Scripps-specific eLearning
