Job Description

This is a casual/per-diem position with 10-hour shifts on varied days of the week, including weekends.

Hours are typically 7am to 5:30pm.

Position is located at Scripps Memorial Hospital La Jolla.

·     Responsible for the daily management and coordination of care for patients in the hospital with Identified complex health needs, including coordination with providers, navigators and ancillary and community resources across the system and care continuum.

  •          Has knowledge of involuntary hold or detention status (to danger to self, danger to others or gravely disabled), conservatorships, public and private as well as Mental Health care needs.
  •          Has extensive knowledge regarding multiple insurance plans operating within the service area and may assist patients in coordinating change in insurance coverages and/or assisting to determine if additional medical insurance coverage or government funding is available.
  •          Assists in coordination and management of patients outside of the hospital for which hospitals may need to follow due to contractual or transition planning post discharge.

Preferred Education/Experience/Specialized Skills/Certification:

  •          BSN or MSN.
  •          Preferred in caring for the acutely ill patients with multiple disease conditions.

 Required Education/Experience/Specialized Skills:

  •          RN
  •         3+ years of managed care and/or case management experience.
  •          Associate degree in nursing.
  •          3+ years of diverse clinical experience.

Required Certification/Registration:

  •          Current RN license in the state of CA
  •          Certified Case Manager (CCM) OR ability to obtain CCM within 12 months of employment or equivalent case management certification and maintained during employment OR 10+ years of inpatient or outpatient case management experience.
  •          Current BLS for healthcare from American Heart Association.

Position Pay Range: $63.55-$92.33/hour

Please also note this range is applicable for employees who reside in California only. A geographical pay differential may be applied for remote employees who reside out of state.
