Job Description
This is a Casual / Per Diem – 10-hour Day Shift. Temporary / Interim position located at Scripps Memorial La Jolla. No weekends or Holiday requirement. Starting in June for about 6 months.
As a Specialty Nurse Navigator, you will be responsible for:
- Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
- Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
- Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
- Identifies and aligns resources that impact high-risk care.
- Coordinates services on behalf of the high-risk patients.
- Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.

Required Qualifications:

Current California RN License
-
Current American Heart Association BLS and ACLS

Three (3) years of recent RN experience in a variety of clinical settings

Knowledge of hemodynamics and experience with advanced mechanical circulatory support; IABP, Impella, Tandem, ECMO, and LVAD
- Bachelor’s degree in nursing (BSN)
- ECMO Specialist Certificate from ELSO (Completion of ELSO Foundations course)
Preferred Qualifications:
- Master’s degree in nursing
- Two years of experience managing care of relevant specialized patient population.
- Evidenced based practice
- Experience educating
- Experience with data registry
