Scripps Mercy Hospital consists of two campuses in San Diego and Chula Vista.
Scripps Mercy Hospital Chula Vista provides a variety of medical and surgical services, including inpatient and outpatient care. Within the hospital you’ll find our 24-hour emergency room and many other specialty services ranging from cancer care to stroke care.

As a Community Health Promoter II, you’ll create a positive Scripps Health experience for our patients while being responsible for the following:
- Identify patients from the emergency department who qualify for Enhanced Case Management
- Connect patients to Enhanced Case Management at local partner sites
- Provide patient support and education which will be offered post discharge to link patients to needed social and medical supportive services.
- Manage documentation as well as maintaining a list of community contacts and referrals
- Implement community outreach and facilitate a variety of community-based education.
- Support a variety of community-based interventions.
- Assist in developing educational content.
Required Qualifications:
- High school diploma or equivalent.
- Excellent communication skills. Demonstrate oral and written communication skills by clear and accurate correspondence, including spelling and grammar usage.
- Demonstrate experience and ability to work independently and as a team player. Ability to set priorities and goals without specific direction. Ability to handle problems, propose solutions, and implement them.
- 1-2 years community-based work experience.
- Verbal and oral presentation skills.
Preferred Qualifications:
- Bilingual in Spanish.
- Basic typing and computer skills.
