We invite you to join our organization, recognized by Healthgrades as one of America’s Best Hospitals for 2019.  The Transition Care Manager will be part of a new care delivery model focused on the CMS Bundled Payment initiative, working primarily with patients targeted for BPCI, and will coordinate with the acute care team to determine appropriate post-acute care plan.  Additionally, the Care Manager will make home visits as needed to ensure appropriate transition of care, and will coordinate with post-acute providers (HH, SNF, LTAC, etc.) as applicable.  The Care Manager will then follow the patients for 90 days post discharge.  This is the ideal role for a registered nurse who is patient-centered, flexible, and results-oriented.   
 
This is a full-time position (80 hours per pay period), with 8-hour day shifts, and may work occasional weekends.  Travel involved as needed, for home visits.    

Job Requirements

Required Education/Experience/Specialized Skills:
  • 3 plus year's clinical experience.
Required Certification/Registration:
  • Current California RN License.
  • BLS for Health Care Provider.
Preferred Education/Experience/Specialized Skills/Certification:
  • Bachelor of Science in Nursing (BSN) highly desired.
  • Case Management and/or home care experience preferred.
  • Case management certification highly preferred.
  • Epic EMR experience preferred.
