As a Social Worker – Population Health you will play in important role within the Scripps Healthcare team and make an impact in the community.  You will be working with the ACO (Accountable Care Organization) and follow a caseload of patients with complex psychosocial deeds in their homes and communities to keep them thriving in their home environment and minimize inpatient or emergency department utilization. Demonstrate ability to perform competencies as outlined in unit specific competency assessment tools. Demonstrate age specific care as established for a Clinical Social Worker in assigned area.
This is a Full Time, 8-hour Day Shift position.  The schedule will be Monday – Friday, with the candidate needing to be flexible to occasional weekends. (64 hours per pay period).  Position is located at our Scripps Office in Rancho Bernardo/4S Ranch.  Candidate will need to have the ability to commute to patient home visits as needed.

Job Requirements

Required Experience/Specialized Skills:
  • Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital.
  • Working knowledge of the laws, regulations, policies and appeal procedures as relative to the Federal, State and other reimbursement programs.
Required Education/ Training/Certification:
  • Masters degree in Social Work from an accredited university or college (MSW).
  • BLS for Health Care Provider required.
  • Valid California State Drivers License
Preferred Experience/Specialized Skills:
  • Licensed by the State of California Behavioral Science Examiners Board as a Licensed Clinical Social Worker (LCSW) preferred.
  • Experience in the hospital setting preferred.
  • An ability to speak another language is desirable.
  • Strong understanding of community resources
  • Psychiatric or Mental Health background preferred
  • Experience using Epic
