We are seeking a compassionate, motivated Home Health Social Worker to join our Scripps Home Healthcare team. As part of this team you will play an important role in supporting our Scripps patients in South County San Diego.

As a Home Health Social Worker with Scripps it will be your responsibility to assist patient and families with personal and environmental problems which predispose illness or which interfere with their obtaining maximum benefits from medical care. In this role, you will be working independently and visiting patients in their home one on one. It will be your responsibility to demonstrate the ability to perform competencies as outlined in unit specific competency assessment tools as well as demonstrate age specific care as established for a Clinical Social Worker in the assigned area. The ideal candidate will be a self-starter, motivated and find reward in being able to function in their role independently with support from the team remotely.

This is a Full Time, 8 hour Monday – Friday day shift position. Candidate will work remotely and support Scripps patients in South County San Diego. Weekly case conferences meetings are held with other members of the team as well as a monthly in person meeting that candidate is required to attend.

Job Requirements

Required Qualifications:
  • Masters degree in Social Work (MSW) from an accredited University/College
  • BLS for Healthcare Provider American Heart Association
  • Valid California Drivers License
  • Working knowledge of the laws, regulations, policies and appeal procedures as relative to the Federal, State and other reimbursement programs
  • Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital
Preferred Qualifications:
  • LCSW (Licensed by the State of CA Behavioral Science Examiners Board)
  • Home Health and/or Palliative Care/Hospice experience
  • Acute Care experience
  • Spanish speaking preferred
  • Proficiency in Microsoft Office including but not limited to Outlook, Excel
