Our department created a Care Manager/Social Work team who carry a case load of our most difficult to discharge patients. With this model we have reduced length of stay significantly. We are building up our connections with the community to coordinate discharges and help with long term solutions for homeless patients. This includes providing grant money to Catholic Charities to assist with case management and hotel vouchers and San Diego Rescue Mission Recuperative Care.
Our Social Worker assists patient and families with personal and environmental problems which predispose illness or which interfere with their obtaining maximum benefits from medical care. Provides peer supervision for other staff, education for social work interns and/or assists with program planning and organization of services. Demonstrates ability to perform competencies as outlined in unit specific competency assessment tools. Demonstrates age specific care as established for a Clinical Social Worker in assigned area.
Full-Time 8 hour shift position with a VARIED Days Schedule - Which means being available to work Monday through Friday, weekends and holidays as needed.
 

Job Requirements

Required Qualifications:  
•         Masters degree in Social Work from an accredited university or college (MSW)
•         Working knowledge of the laws, regulations, policies and appeal procedures as relative to    the Federal, State and other reimbursement programs
•         BLS for Healthcare Provider American Heart Association
•         Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital         
 
Preferred Qualifications:
•         Hospital experience
•         Licensed by the State of California Behavioral Science Examiners Board as a Licensed   Clinical Social Worker (LCSW) ability to speak another language is desirable
•         Ability to speak another language is desirable 
•         Discharge planning and knowledge of managed Medi-Cal and Insurances
