As a Social Worker – Population Health you will play in important role within the Scripps Healthcare team and make an impact in the community. This will be an exciting time to join the organization as Scripps is establishing a new program to assist Medicare patients manage their care post discharge. As the Social Worker of this newly established team, you will play a key role on the care management team as you will work with a select caseload of patients who have complex psycho-social issues and follow them post discharge for ninety days in their homes and communities. You will work closely with the inpatient and post-acute teams to decrease the readmissions of patients and utilize community based resources to help support the patient’s needs. The ideal candidate will have a vast knowledge of San Diego based community resources and has experience working with patients with behavior issues. Looking for candidate who has home patient visit experience.

This is a Full Time, 8-hour Day Shift position. The schedule will be Monday – Friday, with the candidate needing to be flexible to occasional weekend coverage. Position will be centrally assigned to our Scripps Office in Rancho Bernardo/4S Ranch, with candidate needing to have the ability to commute throughout the Scripps Health System. Candidate will need to be open to visiting all Scripps Health sites as well as candidates in their homes and throughout the community.

Job Requirements

Required Experience/Specialized Skills:
  • Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital.
  • Working knowledge of the laws, regulations, policies and appeal procedures as relative to the Federal, State and other reimbursement programs.
Required Education/ Training/Certification:
  • Masters degree in Social Work from an accredited university or college (MSW).
  • BLS for Health Care Provider required.
  • Valid California State Drivers License
Preferred Experience/Specialized Skills:
  • Licensed by the State of California Behavioral Science Examiners Board as a Licensed Clinical Social Worker (LCSW)
  • Experience working in the hospital setting
  • An ability to speak another language is desirable.
  • Strong understanding of community resources
  • Psychiatric or Mental Health background
  • Experience using Epic
