This position is responsible for the daily management and coordination of care for patients with complex health needs, including coordination with providers, navigators and ancillary and community resources across the system and care continuum.

In conjunction with the Medical Director and the Program Coordinator, the nurse VAD Coordinator provides comprehensive management and coordination of care from VAD evaluation through post VAD implant in both the inpatient and outpatient setting. This position also has accountability for patient management, staff education, quality improvement projects and other program oriented tasks.

  • Manages individual patients across the healthcare continuum to achieve optimal outcomes.
    • Pre-implant evaluations and the care of implant candidates.
    • Assists with VAD preparation, startup and monitoring in operating room during implant as needed by the surgical team
    • Inpatient management
    • Provides support to patients during tests and procedures
    • Patient care within an outpatient clinic
    • Monitors patient through remote monitoring
  • Provides VAD education to:
    • Patients and care givers throughout continuum of care- Initial LVAD education, education for discharge, and ongoing outpatient VAD maintenance education.
    • Nursing staff and other health professionals regarding the device and procedure.
    • Community health care professionals who have contact with the patient (i.e. home health nurses, referring physicians, first responders, cardiac rehabs)
  • Participates in and facilitates team meetings including: selection committee, multidisciplinary rounds, patient update conferences, steering committee

This is a full-time, 10 hour, day shift (4 days per week, 80 hours per pay period) position that shares 24/7 call.

Job Requirements

Required Qualifications:

  • Three (3) years of recent RN experience in an acute care setting within hospital environment
  • 1 year experience managing VAD (LVAD) patient population
  • California Registered Nurse License
  • Bachelor's degree in nursing (BSN) or Certified Case Manager (CCM)
  • Registered nursing degree from an accredited school of nursing
  • Current American Heart Association BLS and ACLS Healthcare Provider certification
  • Must be able to speak, read and follow instructions in English
  • Must be able to demonstrate positive behaviors
Preferred Qualifications:

  • Minimum 1 year experience in ICU or CVICU/CTICU
