This RN HBC position is an office based position currently located at 4S Ranch with intent to relocate to Kearny Mesa area by end of calendar year. The ideal candidate will thrive with an integrated team of RNs, LVNs, and administrative staff, and will be able to work and multi-task in a fast paced environment, with good clinical skills to apply to the HH environment. Able to work in multiple systems at the same time,

Provides direct patient care or consultative services that is goal directed within his/her scope of practice. Works cooperatively with patient case manager and all members of the interdisciplinary team including physicians, and other internal and external customers to ensure optimum patient care. Evaluates patients eligibility for services, initiates patient specific Plan of Care.

This is a Full-Time (80 hours per pay period), Day shift position, with a varied work schedule. Will be in a rotation for weekend coverage at every four weeks presently, and holiday rotations. Hours for this position are 8am to 5pm or 8:30am to 5:30pm M-F and 8am to 4:30pm S/Su.

Job Requirements

Required Education/Experience/Specialized Skills:

  • Graduate of Accredited School of Nursing.
  • One year acute care or two years SNF experience.
  • Fluent knowledge of spoken and written English.
  • Working knowledge of state and federal Home Care regulations and standards.
  • Proven ability to utilize a problem solving process and critical thinking skills.
  • Excellent interpersonal and communication skills (able to talk with a variety of customers including patients, physicians, and referral sources).
  • Proven leadership ability, demonstrated time management skills and creatively.
  • Adapts readily to ongoing changes in the health care environment.

Required Certification/Registration:

  • Current RN License in the state of California
  • American Health Association BLS.

Preferred Education/Experience/Specialized Skills/Certification:
  • BSN or PHN
