The Home Based Care RN case manager provides a direct/hands on patient care to the patient and ensures all patient orders are followed completely.  The case manager also oversees and coordinates the total care of the home health patient, including clinical and financial outcomes, from admission through discharge from nursing services. Serves as a resource to other home health/hospice staff. Provides direct patient care within the scope of practice. The case manager works collaboratively with all members of the patient care team to ensure optimal patient care outcomes.  Must have clinical knowledge and will work with patient during patient care while on a home health stay. 
 
Please note this position will work day shift and with a varied work schedule and will rotate weekends every three weeks.

Job Requirements

Required Qualifications:
Graduate of Accredited School of Nursing.
 
One year acute care or two years SNF experience.
 
One year Home Health experience as an RN.
 
Hands on, direct patient care within the last 6 months.
 
Ensures patient orders are followed.
 
Fluent knowledge of spoken and written English.
 
Working knowledge of state and federal Home Care regulations and standards.
 
Proven ability to utilize a problem solving process and critical thinking skills.
 
Excellent interpersonal and communication skills, proven leadership ability, demonstrated time management skills and creatively.
 
Adapts readily to ongoing changes in the health care environment.

Current RN License in the state of California.
 
BLS.

Preferred Qualifications:
BSN.
 
Home Health case manager experience.
