Expect shift: Monday - Friday 7:30-4:30.
Responsible for assisting care team (medical assistants providers ) through primary care redesign and tracking patient lists (hypertension, diabetes, chronic care, medicare questionnaires, monthly quality checks, Lawson orders, etc.) in addition to standard daily work.
Performs patient assessments and care tasks, including emergency procedures and medication administration as appropriate. Educates patients and documents patient interactions. Practices infection control. Contributes to the team as necessary to ensure quality patient care within own scope of practice

Job Requirements

Required Education/Experience/Specialized Skills: Completion of accredited LVN program

Required Certification/Registration:
  • Current LVN CA license
  • BLS certification for the Healthcare Provider by the American Heart Association (AHA)
Preferred Education/Experience/Specialized Skills/Certification: Minimum of one year outpatient experience preferred
