As Complex Care Manager RN, supporting Scripps Health Plans, you will be responsible for the daily management and coordination of care for patients with complex health needs, including coordination with providers, navigators and ancillary and community resources across the system and care continuum. This position will provide telephonic care management, helping patients through the process and coordination of their care. You will provide scheduled support to the prior authorization team for after-hours call. You will also notify the doctor of any problems or changes and maintain communication with patients and their families.

This is an 8 hour, day shift, full-time (80 hours per pay period) position with benefits, located in 4S Ranch.

Job Requirements

Required Experience/Specialized Skills:

  • Three (3) years of recent RN experience in a variety of clinical settings.
  • Two (2) years of experience managing care of relevant specialized patient population.

Required Education/Certification/Registration:

  • California Registered Nurse License.
  • Bachelor's degree in nursing (BSN) or Certified Case Manager (CCM).
  • Registered nursing degree from an accredited school of nursing.
  • Current American Heart Association BLS.

Preferred Requirements:

  • Behavioral Health experience
  • CCM certification
  • Hospital case management or ambulatory complex care management experience preferred.
  • Familiarity with NCQA requirements
  • Spanish speaking preferred.
