Job Description

The case manager oversees and coordinates the total care of the home health patient, including clinical and financial outcomes, from admission through discharge from nursing services. Serves as a resource to other home health/hospice staff. Provides direct patient care within the scope of practice. The case manager works collaboratively with all members of the patient care team to ensure optimal patient care outcomes.

Job Requirements

Required Education/Experience/Specialized Skills:Graduate of Accredited School of Nursing. One year acute care or two years SNF experience. Fluent knowledge of spoken and written English. Working knowledge of state and federal Home Care regulations and standards. Proven ability to utilize a problem solving process and critical thinking skills. Excellent interpersonal and communication skills, proven leadership ability, demonstrated time management skills and creatively. Adapts readily to ongoing changes in the health care environment.

Required Certification/Registration:Current RN License in the state of California. BLS.

Preferred Education/Experience/Specialized Skills/Certification:BSN. Home Health case manager experience strongly preferred.
