General Description
The Primary Care Advanced Practice Clinician (APC) works closely with and is supervised by the physicians in a team-based setting in Internal Medicine and Family Medicine. The APC is expected to have the highest standards for clinical capability, professional education, quality improvement and overall patient care. The position requires a current California Nurse Practitioner or Physician Assistant license and DEA furnishing certificate.  Full time is 40 hours per week
 
 
Key Responsibilities
 
  • Assesses patients with numerous complaints independently and in collaboration with the supervising physicians
  • Participates in acute and chronic care management of patients, specializing in assessment and documentation of high risk conditions
  • Performs screening for high risk conditions
  • Performs history and physical assessments
  • Performs minor procedures – after needed training and proctoring is completed for the procedures
  • Orders and manages laboratory and radiographic findings
  • Writes medication orders on patients and prescribes medications
  • Provides patient education to all patients
  • Provides phone triage
  • Participates in population management to improve the health of patient panels.
  • Participates in telehealth and telephone visits
  • Participates in IS and Risk Adjustment program activities and meetings.
  • Participates in conference and related continuing education
  • Participates in training on risk adjustment workflows and protocols as needed.

  • Up to 40% (approx. 4 session per week) assigned for Same Day/ Team-based Primary Care access
  • 60% Risk Adjustment Program
    • Pre-Visit Planning (CDI)
    • Enhanced Encounters
    • Program Development/Meetings – for example, developing clinical skills and protocol/standard workflow to address patient care gaps
    • Peer to Peer training
